MediLink Homecare, Inc.’s clinical pharmacists act as case manager for the patient. The pharmacist engages in cooperative decision making with the physician, patient, caregiver and payer to ensure appropriateness, safety and efficacy of medication management.

The initial consultation assesses health and medication history. On going communication addresses issues related and unrelated to current therapy. The patient is counseled regarding appropriate dosage, administration, allergies, possible side effects and expected outcomes.

Physicians rely on the clinical monitoring we provide. Monitoring lab results, adherence to regimen, dosage adjustment recommendations and formula calculations of TPN are a few of the cognitive services routinely performed.

A pharmacist is on call 24 hours a day 7 days a week to consult with you.
